EFICACIA DE UN PROTOCOLO PARA LA PREVENCIÓN DE INFECCIÓN DEL DRIVELINE EN PACIENTES PORTADORES DE ASISTENCIA VENTRICULAR IZQUIERDA

Resumen

Introducción: Las infecciones del driveline representan una de las complicaciones más frecuentes en pacientes portadores de dispositivos de asistencia ventricular izquierda, con un importante impacto en la morbilidad y la calidad de vida. El objetivo de este trabajo es evaluar la eficacia de un protocolo en la prevención de la infección del driveline.


Metodología: Análisis retrospectivo de los pacientes con dispositivo de asistencia ventricular en un hospital universitario terciario entre septiembre de 2011 y marzo de 2025. Se recopilaron datos sociodemográficos, clínicos, y porcentajes de infecciones. La recogida de datos se dividió en dos periodos: el primero (2011-2017), previo a la implementación del protocolo; y el segundo (2018-2025), tras su incorporación. Se realizó un análisis descriptivo frecuencial.


Resultados: Se incluyeron 17 pacientes ,15 hombres y 2 mujeres. La edad media fue de 64,7 años (Intervalo de confianza 95% 61,6-68,3). De los 8 pacientes incluidos en el primer periodo, 3 (37,5%) presentaron infección bacteriana local en los tres primeros meses del implante y además, uno de ellos (12,5%) tuvo una infección fúngica a los 5 años. En el segundo periodo, se incluyeron 9 pacientes. Ninguno presentó infección tras la implementación del protocolo. Actualmente 5 pacientes continúan en seguimiento ambulatorio, todos ellos libres de infección.


Conclusiones: La implementación del protocolo ha demostrado ser eficaz en la disminución del número de infecciones. Destaca la importancia de su adherencia, el seguimiento continuado por la enfermera coordinadora de DAVI y la optimización del procedimiento de cura.
