DIFERENCIAS SEGÚN SEXO EN LA SINTOMATOLOGÍA Y LA CALIDAD DE VIDA DE PACIENTES CON FIBRILACIÓN AURICULAR SOMETIDOS A UNA ABLACIÓN POR CATETERISMO

Resumen

Introducción: La fibrilación auricular (FA) es la arritmia más frecuente y se asocia a una importante carga sintomática y deterioro de la calidad de vida (CdV). La presencia de síntomas y su impacto funcional constituyen una de las principales indicaciones para la ablación con catéter. Sin embargo, existe escasa evidencia sobre posibles diferencias según sexo en estos pacientes.


Objetivo: Describir la sintomatología y la CdV según sexo en pacientes con FA remitidos para ablación con catéter.


Métodos: Estudio prospectivo, transversal y unicéntrico realizado en un hospital de tercer nivel entre enero/2021 y marzo/2022. Los pacientes evaluados en consulta preablación completaron el cuestionario ASTA (Arrhythmia-Specific Questionnaire in Tachycardia and Arrhythmia), instrumento validado para evaluar síntomas y CdV en arritmias y se analizaron las diferencias según sexo. Se consideró significativa una p < 0,05.


Resultados: Se incluyeron 110 pacientes (60±11 años), de los cuales el 32% eran mujeres. Las mujeres presentaron mayor intensidad sintomática (8,8±5,8 vs. 6,2±4,9; p=0,024) y peor CdV (14,4±9,2vs.9,4±7,5; p=0,006), con diferencias significativas en los dominios físico (p=0,004) y mental (p=0,022). La frecuencia y duración de los episodios arrítmicos fueron similares entre sexos.


Conclusión: Aunque representan una menor proporción de los pacientes remitidos para ablación, las mujeres presentan mayor carga sintomática y peor CdV relacionada con la FA. Estos hallazgos subrayan la importancia de incorporar la perspectiva de sexo en la evaluación clínica y pueden contribuir a orientar decisiones terapéuticas más individualizadas en el manejo de estos pacientes.
